Our teams of professionals understand the difficulty in making the right placement decision for your family member. Nurse Liaisons are available to meet you in your home or hospital to assist with coordinating care of your family member pre and post hospital, to ensure a smooth transition to the health care center.
Admission teams are available 7-days a week to accommodate individual needs for admission and after hours inquiries. These teams are knowledgeable and will assist you with Medicare, Medicaid, insurance processing and related issues.
